Our patient-centered care enables the skilled nursing team to develop personalized care plans to address each resident’s physical, spiritual, social and emotional well-being. Services are directed by an executive director, nurses and a medical director who specialize in geriatric medicine. Patient-centered care coordinated by registered nurses, certified nursing assistants, therapists, social worker, activity staff and dietitians.
